目的：调查住院手术治疗的老年非黑色素瘤性皮肤恶性肿瘤(non-melanoma skin cancer, NMSC)患者的临床流行病学特征。方法：收集南通大学附属医院2012年2月—2019年12月住院手术治疗的老年NMSC患者的临床资料，回顾性分析其性别、年龄、发病部位、病理类型等分布情况，并统计此类患者的医疗疾病负担。结果：共收集老年NMSC患者736例，其中男327例，女409例，男女性别比为1∶1.25；年龄60～101岁，男(74.93±8.65)岁，女(79.56±8.59)岁。不同年龄组男女性别构成比差异有统计学意义(χ2=50.99, P<0.001)，其中≥80岁女性患者显著多于男性。年龄高峰为70～<90岁，共507例(68.89%)。发病部位以头面部为主，共557例(75.68%)。病理类型以皮肤鳞状细胞癌(squamous cell carcinoma, SCC)最多，共467例(63.45%)，其次为基底细胞癌(basal cell carcinoma, BCC) 211例(28.67%)。住院时间分布中位数为6(5, 8) d，直接住院医疗费用为10 411.99(2 100.30, 30 085.66)元。结论：NMSC在70～<90岁老年人群中疾病负担尤为沉重，亟须对该群体实施早期筛查与综合防控策略，以降低住院手术率并改善其预后。
Abstract：Objective: To investigate the clinical epidemiological characteristics of elderly patients with non-melanoma skin cancer(NMSC) who underwent inpatient surgical treatment. Methods: The clinical data of elderly patients with NMSC who were hospitalized in the Affiliated Hospital of Nantong University from February 2012 to December 2019 were collected, and the distribution of gender, age, location and pathological type were retrospectively analyzed, and the medical disease burden of such patients was counted. Results: A total of 736 elderly patients with NMSC were collected, including 327 males and 409 females, with a gender ratio of 1∶1.25. The age ranged from 60 to 101 years, the average age was(74.93±8.65) years for males and(79.56±8.59) years for females. There was a significant difference in the gender composition ratio between male and female in different age groups( χ2=50.99, P<0.001), in which female were significantly more than male in patients≥80 years old. The peak age was 70-<90 years old, a total of 507 cases(68.89%). 557 cases(75.68%) were mainly located in the head and face. The most common pathological type was squamous cell carcinoma(SCC) with 467 cases(63.45%), followed by basal cell carcinoma(BCC) with 211 cases(28.67%). The median length of stay was 6(5, 8) days, and the direct hospitalization medical cost was10 411.99(2 100.30, 30 085.66) yuan. Conclusion: NMSC has a particularly heavy disease burden in the elderly population aged 70-<90 years, and it is urgent to implement early screening and comprehensive prevention and control strategies for this group to reduce hospitalization surgery rates and improve their prognosis.
